Request for Payment Plan

 A payment plan will close as a conviction on your driving record. Payment plans are available for a maximum term of eight (8) months, structured as monthly installments. You have the option to pay the balance off in 30 days, otherwise a $15.00 time payment fee will be assessed on the 31st day.

This request form must be filled out in it's entirety and signed by the defendant. A copy of your driver's license or photo ID will also be required when submitting this form.

Waiver of Trial and Request for Payment Plan
